D4 with flat spot
 

We have a 2015 S60 D4 at present which has a flat spot from stand still and then again on constant throttle. Any ideas? We have replaced maf sensor and inlet manifold so far but also keeps throwing up different error codes all the time. Any help will be welcomed.
