Sure I just forgot to mention it might be a slow/stuck EGR valve as well. Especially since it occurs just after gear shifts. When you press the clutch the engine is not under load so it opens the egr valve to recirculate the exhaust flumes but then when you hit the throttle again it doesn't close properly so you experience a lack in power. You won't necessarily experience this pulling away from standstill because the EGR should be closed during idling
You can check the actual EGR position vs the commanded position which should follow each other pretty closely. I would expect an error message though if this deviate to much, but that is just my opinion.
