Oh...and the double quoting problems are people getting a bit over-active with the delete button.
Where you've got stacked quotes....e.g.

...you need to open each quote with a [ quote] (without the space), and close it with a [ /quote] (again, sans space). The second [ /quote] gets lost very easily.

...without the spaces!
