Seems like its been covered in the above suggestions.
Sounds like yours is re-mapped with a 1st gear limit.
Personally, I'd get a smoke machine, connect it right after the MAF and test for leaks. Quick and easy.
Second, adjusting the actuator is easy as well and IPD volvo has a video on it.
MAF and TCV are possible culprits as mentioned above but, are easy to test/replace.
