13.8 v is too low for charging it , it should be around 14.2 or a little more unless you mean that is the voltage after the battery has been standing un connected overnight?

Yes the alternator is direct gear drive from the crankshaft , there is a rubber coupling which was supposed to have a change interval of 144000 miles , but with the passage of time and miles they have now said it does not need changing ..
