That is known as intercoat adhesion failure and as has been said is down to poor prep, cleaning or incorrect mixing/wrong type of paint. I've seen a fair few cases a few years/months after "smart repairs".
As said above - the number of cars that have paintwork before the first owners see them is probably more than you think.
I would say 99.9% that is not the original factory paint.
