I suspect that quite often it isn't the system itself which is at fault.
Many systems are mechanical, and robust and not likely to fail.
Rather it's the sensor or warning system which is faulty as they are electronic and computerised, and like any computer or electrical components far more likely to fail.
Replacing the sensor usually resolves the "problem" - one which didn't actually exist in the first place.
