I changed the discs, pads & shoes a couple of years ago and handbrake was OK but gotten progressively worse in the last few months, eventually ran out of adjustment. Checked the shoes and still fine with plenty of friction material left and drum surface fine. Couldn't find any obvious wear on anything (including compensator & handbrake lever) so decided that 11 year old cables on 132k MY2004 D5 might be stretched. Changed cable to each wheel and no improvement, still on adjustment limit.
Found this thread so decided to change to adjusters on the rear and replaced the expanders at the same time (total cost around £50 - expanders from PFS, adjusters from Dealer).
Followed the install / adjustment methods above and now have a very effective handbrake - for the first time in 7 years of ownership.

Can anyone clarify exactly what is needed .So, the H bolt is replaced with an adjustable one. Is there another part with it that needs replacing to have and adjustable setup? I'm a bit confused about what part no's 17 and 18 is are the diagram.

The H part is just a flat plate, not a bolt. It is replaced by an adjuster. If you get the Bremtech adjuster from here, it includes the adjuster and the so called 'claw', which is simply the other end of the adjuster:
