Default V40 Bore Plug - why should it fall out?

I have a 2001 V40 which has just been returned to me by Pilling Volvo in exchange for £160.

The reason for this being that a bore plug which was situated on the block below the exhaust manifold decided it was time to fall out onto my garage floor.

Although I didn't see it as I reversed from the garage, as soon as the engine warmed up I could smell oil. I had only travelled about a mile so I promptly took her home. My local Volvo dealer advised me to get it 'relayed' to them rather than risk damage by driving the 11 miles.

As I have only done 35,000 miles, is this reasonable to expect bore plugs to simply fall out, or should I complain to Volvo UK?

Default core plugs

they are called core plugs and unless the engine had been frozen due to lack of antifreeze they should never ever come out so must have been fitted incorectly or the engine had been machined wrong
