One point about air leaks into the fuel system is that although it often resulted in starting problems with older types of diesels, it seems to result in stalling with more modern ones. My local Indy forgot to fit one of the fuel filter seals, it started and ran OK, but everytime I lifted my foot off the go pedal, it stalled.

The MAF sensors do seem to cause a few issues, so I think it might well make sense to remove it and use a good quality MAF spray (Liqui Moly or CRC) when changing the air filter. The air filter seems to last around 40K km, so that's not going to be much extra work.

I do tend to like using some engine cleaners, so I currently use Diesel Purge when changing fuel filters and if I was not convinced the block was clean, I would be using an idle flush just before an oil & filter change (Don't use a flush if you have an oil leaks), so I might as well start using a MAF spray when changing air filters.

Unfortunately there is no spray or additive that will keep an EGR clean, so I'm lucky that the occasional high power session on the autobahn seems to be doing that job for me.

Cheers all, car is going in this week for EGR clean out, I'll get him to give the MAF a clean with MAF cleaner, and if that doesn't help, I will swap the MAF over, and then check the N75 solenoid for vac pressure.

On the N75 point, given the strange behaviour of mine under throttle, should I be expecting no vac at any revs, or only when I boot it and experience the issue? If the latter it will be very difficult to test as it mainly only happens when I'm booting it under load (i.e. when I'm doing 50mph in 3rd )

Will also give pour-in injector cleaner a go, as suggested, but first things first, don't think the EGR has ever been cleaned.

My N75 solenoid, had no vacuum even when idling. When I replaced it with a spare one, I was getting vacuum on idle. No need to check it when revving the car

After EGR clean the flat spots are much less pronounced, although still just about there, and I now get fairly smooth acceleration all the way up the rev range. Big improvement but it has just made it more obvious that the engine is quite old and not as powerful as my last car. Or maybe it WAS mapped/chipped after all...

New MAF and N75 next on the list to try.

Only issue you have is you can pass anything except a petrol station
