Default Car Over-revs between 1st & 2nd gear

I've just bought my first Volvo (2004 S40 2.0D SE) and it's got a few problems. The main one is the accelerator, which over revs by itself 500-1000rpm when I accelerate hard in 1st gear and change up to 2nd gear. This means I have to change gear at 2,000 revs which makes for very sedate driving. Where is the problem, is it the throttle position sensor or the accelerator pedal, or can it be fixed by just cleaning the throttle body?

The second problem is smoke, it pumps out black smoke on acceleration in any gear.
