the oil pressure is measured with a oil pressure tester, expensive kit from work what is used on plant machinery .
The pressure is taken from the oil pressure switch take it out and screw in the fitting then attach the gauge.
The oil is 5-30 the pressure at cold was 85-90 psi once warm it was @70 psi I took some oil out and it was at 50 psi oil does matter cause if it was over filled then the engine pressure would pop the crank seals, so fare enough your saying 50 psi is fine relating to oil pressure ,I've had the motor a very long time and I know my engine the clattering is not normal maybe to you but certainly not to my engine why do you think the lubrication and breather system was serviced because of the chattering from head then the engine pressure was tested and believe the pressure is high and not feeding enough oil to top end
try an engine flush as it can be a build up of carbon on your piston and piston rings.

From what i have read , i would base your problem on debris moving around the oil galleries . It has then either jammed the pressure relief valve closed causing pressure to exceed working limits or passed that & reduced the flow along the oil ways . However your rattly top end would again point to debris blocking oil galleries & starving the valve train of the volume of oil it requires to keep the hydraulic followers pumped up . Engine flush may help but it is very unlikely . The last engine i saw with this problem was an industrial unit running 24/7 with extended periods between oil changes , this led to carbon build up that broke away causing all the symptoms you have . My solution was to rebuild engine after " hot tanking " in caustic solution to remove all deposits . It really is the only way to ensure all deposits are disolved & removed , some people will root about with very fine bottle brushes to clean gallries out , but you run the risk of crud breaking free with the engine running & seizing . You also need to power flush every gallery to force out any remaining junk . I aimed steam cleaner into where oil pump fitted & into oil filter fitting and any other oil outlet i could find

To be honest whilst the oil pressure does sound a bit high it wouldn't cause the top end to clatter although a sticky relief valve could cause this.
The possibility of a partially blocked oil way isn't out of the question.
If the clatter is bad then removal of the top cover and check the lobes of cams and tappets/buckets to see if there running dryish.

I'm on with building back up the top end after inspecting the cams ,valve clearance ,oil galleries, there is no evidence of oil starvation on the cam lobes or bearing cover the valve clearance on all inlet and exhaust are excellent . But after inspecting the camshaft seals they are all leaking slightly and the inlet front seal looks like it's been slipping around as there are marks on the cam bearing cover so in relation I believe that the camshaft cover wasn't down enough on torque I've bought the volvo special tools now that clamp the cover down so I'm putting it back on today
