Default Failed aux belt tensioner

Having had the aux belt tensioner replaced under recall, 6 weeks and I'm guessing less than 1000 miles later it's just failed. Luckily not taking the engine out with it. Looking at it the bolt that holds it on has sheared off. Anyone else had this? (2011 S60 d3 163)

The recall removed the old hydraulic tensioner to fit the more reliable?! spring version. Given the time and miles passed I'd assume the garage should pick up the tab, or am I in for a shock?!

Just waiting for recovery...

I am puzzled as to how the bolt has sheared. A new bolt comes with the tensioner and torque setting is only 25/30nm if I remember correct. It would suggest it has been fitted incorrectly and they should rectify it

Looking at the bolt the thread lock looks undisturbed for a portion of it which is suspicious as if it wasn't done up all the way/properly. Just trying to work out if the tensioner attaches to the block or a mounting plate. If the block it's going to be messy to sort out.

Realised I posted in general rather than the S60 sections, oops. I blame sitting in the cold...

It bolts directly to the block with a locating peg no metal plate. Has the thread in the block become damaged or the surrounding area?

The surrounding area looks slightly damaged. Looking at the bolt the thread lock on it looks undisturbed for part of it. I'm assuming it was fitted right. I photographed all the parts.

Car is now at the garage but no timescale on it as they are busy, but would endeavour to look at it as they need to determine why it failed. Not overly impressed by the service, I have no car and no courtesy car so can't get to work. I will be chasing tomorrow.
