Check for leak-back on the fuel injectors the cheapest and easier thing to do first. If one or more of the injectors cannot hold pressure then the rail will not be able to maintain the pressure required to start or run.

I'm not over familiar with the Volvo set-up but drawing from all the problems I had with my Chrysler diesel engine you may need to bleed the system and if the pump is not capable of lifting there may be a lift pump taking the fuel from the tank to the high pressure pump. Also check all the filters if you haven't already and the high pressure pump may also have a proportioning valve or variable fuel pressure valve designed to release excess fuel back to the tank; this may be failing or the seals may be leaking. These are some of the problems I had with the BOSCH pump on my Chrysler.

Remeber, everything with diesel is a bit more problematic than petrol in my opinion; you are dealing with fuel pressures that are greater than the pressure at the depth the Titanic is at!
