Default Is this a noisy tappet in my Volvo V40?

Hi, my 2001 V40 Diesel D4192T3 engine sounds like this:
Noisy clicking engine!
This was after a turbo failure which probably resulted in some oil starvation. I've since had a refurbished turbo fitted. Thinking that it might be a sticky tappet I took the camshaft cover off today to measure the valve clearances (using the instructions from the Mitsubishi manual for the F9Q). These were my measurements:
Cylinder 1:
Intake 0.13mm
Exhaust 0.35mm
Cylinder 2:
Intake 0.15mm
Exhaust 0.39mm
Cylinder 3:
Intake 0.15mm
Exhaust 0.40mm
Cylinder 4:
Intake 0.09mm
Exhaust 0.37mm
Cylinder 4's intake is well out of bounds (cylinder 1 is also out but only just). What I can't square up in my head is how having a tappet that is closer to cam can result in a such a tapping sound. I was expecting a tappet to have dropped from having no oil underneath it, not to have risen!
Additionally I tried the test of putting a stethoscope on the cover to try to determine which tappet might be making the noise. To be honest from that test I'd say the noise was loudest when I listened to the cover of the vacuum pump rather than listening to the camshaft cover but it does seem to be coming from the cylinder 4 end.

Anyway, my question is does this sound like a tappet that has risen to you? How does a high tappet make a noise like I'm hearing? Or is the noise something completely different?

wow thats certainly a knock , tappet clearances are set for life and only need attention if something is done to the valve gear . your gaps are not bad .
That is a heavy knock though probably not tappets as such , it may be a broken valve spring . Can you run it without the cover ? it may be a bit messy but could help pinpoint the exact valve causing the noise , hint , a piece of hose a couple of feet long will help if you listen down the top and move the far end around each valve in turn .

Hi. Thanks for the suggestion of listening through a hose; I spent this afternoon giving it a go. I also ran it without the camshaft cover in an attempt to pinpoint the noise. Unfortunately, to be honest I'm not much clearer. I reckon the noise is distinct from the chatter of the cams hitting the tops of the tappets. Other than that it is somewhere between cylinders 2, 3 and 4. The problem is it is so loud it is really difficult to narrow down! At one point I was sure that it was coming from below the head around cylinder 2 but it reverberates through the metalwork so much it could be anything. I think that it might be slightly quieter when I’m engine braking but this might just be the background noise levels being raised. I noted that cylinder 3 leaks slightly around the glow plug (oil from the cam splashed into the cup of the glow plug shaft) and additionally removing the injector lead for that cylinder doesn’t seem to upset the running of the engine so much as removing one of the others.
Anyway, I decided that I’m just going to live with it for now. I haven’t got the money to get it into a garage or the time or resources (or skills) to take the engine apart looking for it. If the car lasts out for a couple of months then I should be in a position to get it professionally checked out or ditch it and get myself something else as there is a number of things starting to go wrong with it now.
