V40 2.0 turbo suspect head gasket or coolant issue

Hello. I need some help diagnosing a problem please. Here's the symptoms and what I've tested. Any advise and thoughts appreciated

Symptoms - starting to struggle to start turns over fine but sometimes need to turn over for more than normal to get going. Seems to drive ok still
Losing or using coolant. No obvious leaks
Temp guage goes to the right position but goes down to quarter and fluctuates when you drive steady
Has boiled the water out the header tank when left idling for 15 mins
Blowing a bit out the oil filler cap- don't know if thinks normal
Radiator fans cutting in when engine is cold
Once engine is hot the revs stick at 2000 or 3000

Tested
Thermostat is ok
Header tank has been tested for head gasket and no problems shown
No calcified oil on dipstick or oil filler cap

Sounds like temp sensors faulty

I would guess the waterpump :not pushing the water around than boils out at the filling reservoir or at the drain from the pump that leaks.
This does not show it runs over hot engine.
When driving there is not always water at the temp contact so temp fluctuates

I had some troubles with the rad fan resistor a while back. Car would be fine at high speed but if I got stuck in traffic the temp would go right up. Replaced the resistor and all was well. There is a thread somewhere on here That provides more detail. Let me know if you need help finding it. Easy enough job to replace.

That's a known issue, they even made a bulletin about.

Just thought I would update the thread. I have replaced the fan sensor which has cured the fan issue. Also replaced the thermostat although it tested working ok once the fan was sorted and I drive it the temp gauge was going down. Symptoms of thermostat stuck open. Now the gauge stays where it should be. Hasn't boiled the water again so my theory is maybe it was stuck closed and freed up when it was taken out to test.
