10 years or 54k doesn't sound right to me . Its recommended to change the aux belt every 54k & the tensioner every 10 years. The cam belt is also recommended to be replaced every 10 years or 108k, so on the big 10 year/108k service, the cambelt should be replaced with the aux belt & the tensioner.

If your tensioner has already been replaced four years ago with a new aux belt belt (Presumably?), then you could probably just replace the aux belt at the five year mark & replace the tensioner when that's due in 2026, if you wanted.

Personally though, I'd replace the tensioner at the same time as replacing the aux belt... 10 years on a tensioner just seems too long on a part that is known to fail, but this is my opinion. Yes it may seem over the top changing the tensioner every five years, but its a £60 part... Its good peace of mind. A failed tensioner or aux belt can potentially cause serious damage. The cost of a £60 part over five years (£12 per year) wouldn't even buy you a pack of decent brand cigarettes nowadays, so I don't consider it to be expensive.

For that reason, in my opinion, its a no brainer to replace the tensioner every five years at the same time as the aux belt. It would be much more expensive to rebuild the engine if either of these parts fail, put it that way.

Your car & your choice though, of course.
