Just to confirm, you are downloading the maps to the USB drive using the Sensus Update application, having downloaded and installed the application from the Volvo Maps page?

Do not just drop the download onto a usb it will not work, click on it and use the volvo download tool which will put it on the usb stick in the correct format for the car to read.
You may have to scroll up or down after you click on the download to see the tool.

when you copy files from the hard drive to a usb stick the files must be copied to the top level folder and not to any sub folder structure. For any particular area there should be a database info fileo plus several zip files

Correct, as here
EU_UR150_GB-IE-IS_1296.DBINFO
EU_UR150_GB-IE-IS_1296.zip
EU_UR150_GB-IE-IS_1296_of_02.zip

You should just plug the usb into your car and it will then install the update. You don't need to do anything else.
If you turn off the car and lock it, the installation will pause until you start again. May take a while at the moment.

For my future reference as I have not had to do this yet (and excuse any technology ignorance on my part!), I assume you have to leave the USB plugged into the car for it to continue to update next time you use it?

Yes, plug it in, then leave in place untill the update is complete. When you first unlock the car, and the sensus screen loads, you will get a message across the top of the screen saying "Installing updates". You might catch the message later on when it has finished updating. I usually look for the "Installing updates". If that doesn't come up, then I assume it has finished. It doesn't matter if you leave the stick plugged in.

Yes, just leave it plugged in to the USB port with the white ring around it and the next time you start the car it will continue from where it left off.
Once the load is complete (100%) you will need to stop the car, turn off the ignition, remove the USB. The app will then load onto the satnav system is when you restart the car.
