First thing that springs to mind is a lazy fuel pump. The fuel pump on the T4 tend to become lazy at approximately 100k miles. When it occurred did it happen to be low on fuel by any chance?

The other thing to check is for hairline cracks on the insulator of each coil pack (they can arc against the head) and that the springs in each insulator are making good contact with the top of the spark plugs. They can be stretched/pulled out slightly.
