Symptoms were:

- Cooling system was retaining pressure. Even when the engine had been cold for a couple of days, removing the expansion tank cap would lead to a 'pop' and coolant spraying out.

- Losing a small amount of coolant.

- There did seem to be more white vapour from the exhaust than normal.

- Evidence of coolant escaping from pipe joints and also around the head.

- When having been run, engine bay would smell of coolant.

However, the engine always ran at normal temperature. Just before it was diagnosed, we had driven a 140 mile round trip without any issues. Also, there was no evidence in the engine of water and oil mixing.

Volvo wanted £20 for a replacement cap but I managed to find a Febi branded one on eBay for £5.
