I have the Volvo accessory, Bluetooth Hands Free System.

Links through car radio. Excellent sound quality adn clarity . Voice recognition dialling.

It has just come out of my 2008 V70, registered Jan 2008.

Cost £350 when fitted new. On sale for £100

Is it model specific, or will it work on a 2004 XC90 with the premium Dolby Sound system?

its sound system specific and model specific due to the plugs on the speaker wiring , There is a basic kit and a kit specific for your model and sound system to connect the basic kit to your car ... you could buy it and buy the fitting kit for your XC90 from Volvo .. but at £100 you need to have a 2008 V70 with the particular sound system of the seller .
